Vivere Health offers benefits options designed to meet the varying needs of our diverse workforce. Employees can select the plan and coverage level that best meet their financial and family needs, including coverage for same-sex domestic partners and their children. Full-time employees become benefits-eligible on the first of the month following date of hire.

Health Insurance

Two Preferred Provider (PPO) health plan options are currently available:

  • $1,000 deductible for in-network care, 80%/20% coinsurance after deductible
    • Preventive care not subject to copay, coinsurance or deductible
    • $20 primary care/$40 specialist copays
    • $10/$35/$50 pharmacy copays
    • $2,500 individual/$5,000 family out of pocket maximum
  • $2,500 deductible for in network care, 100% coverage after deductible
    • Preventive care not subject to copay, coinsurance or deductible
    • Health Savings Account (HSA)-eligible
    • Employer contribution for employees who contribute to HSA

Dental Insurance – Dental flexible PPO

  • $50 individual deductible
  • Preventive dental care covered at 100%, not subject to deductible
  • In-network basic dental services (including endodontics & periodontics) 90%/10% coinsurance after deductible
  • $1,000 annual benefit limit

Vision Insurance Vision PPO

  • Annual in-network vision exam – $10 copay
  • Annual lenses (glasses) – $25 copay
  • Every 2 years frames – covered up to $120, then 20% discount

Long Term Disability (LTD)

  • Company-paid benefit
  • Replaces 60% of monthly salary, up to $6,000
  • 90-day elimination period

Life and Accidental Death & Dismemberment (AD&D)

  • Company-paid benefit = 1x annual salary, up to $100,000
  • Additional voluntary life coverage available
  • Spouse/Child(ren) voluntary life coverage available

Flexible Spending Accounts

  • Employees may contribute pre-tax income to flexible spending accounts which can be then used for IRS-approved Medical expenses, or Dependent Care expenses.

Voluntary Benefits

  • Employees are offered a number of voluntary benefits, through vendors such as Aflac, which, if selected, may be set up on a payroll deduction basis.

Paid Time Off (PTO)

  • Accrued time off benefits allow employees to access paid time when off work for personal reasons, illness and vacations. New employees accrue PTO at a rate of 4.31 hours/biweekly pay period = 14 8-hour days annually. Employees may “carry over” up to 40 hours of accrued/unused PTO from one calendar year to the next.

Paid Holidays

  • Vivere Health observes the following days as company-paid holidays: New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving (Thursday and Friday), Christmas Eve, Christmas Day. Employees who are scheduled to work on a company holiday will receive 8 hours of holiday pay, plus time and one-half the regular hourly wage for any hours actually worked.
